1.Must provide a copy of Drivers License 2.Social Security 3.Alien Registration Card 4.Truck Registration 5.Must be at least 23 years of age 6.Must have three years verifiable experience 7.Must have current physical and supply a copy of the long form 8.Must have a valid CDL in the state of residence 9.Must not have a DUI in the previous five years 10.Drivers will be responsible for paying their own drug screening 11.Drivers will have to be approved by OMA Insurance Company
